Migraine headaches and their surgical treatments
Migraine headaches don’t always originate from the same place in the head or the neck. In fact, many patients report being unable to pin point the exact location of the pain. This makes it challenging for medical professionals to understand the nature of the pain. This is where we recommend our patients to first try our diagnostic treatments such as the Botox treatment and the Nerve Block treatment. The different approaches of the migraine relief surgery are used depending on the location and focus of the headache. The location of the headache impacts the pain and the development of other migraine-associated symptoms. Knowing where the problem lies helps in creating a treatment plan that is focused and beneficial.
Migraine headaches can occur in these regions:
Forehead And Brow Regions
For headaches focused in the brow or forehead regions, the muscles known as the corrugator supercilli are gently removed. Dr. Rosenberg also looks for other sites of compression or damage to the supraorbital or supratrochlear nerves, which may be related to the triggering of migraine headaches.
The occipital nerves run around the head, from the back of the head to the forehead and around the ears. They connect the spine and upper neck to the brain. Occipital migraines are a result of irritation or injury to the occipital nerve. For occipital headaches, a small incision is made over the back of the head, which is hidden within the hair. The greater occipital nerves are located and decompressed and in some cases the lesser and/or 3rd occipital nerve is also treated.
Just like occipital migraines, temporal migraines may result in patients developing extreme sensitivity to lights & sounds and may lead to other migraine-related symptoms. For temporal headaches, Dr. Rosenberg locates the zygomatico-temporal nerves, which are resected and buried in the temporalis muscle.

Recovery and post-surgical care
Recovery from the headache surgery with Dr. Rosenberg varies depending on the area which is being operated on. Here are some treatment details you need to be aware of:
- In the brow or forehead area, swelling may persist for as little as 2 weeks to as long as 6 weeks, depending on the individual.
- Swelling from surgery in the temporal or occipital regions (side and back of the head) is much less noticeable since it is typically hidden by hair. You may shower and wash your hair two days after surgery and thereafter.
- Pain relief medication may be recommended depending on your requirements. We advise that you do not take any medication without prescription from Dr. Rosenberg.
As with all surgeries the migraine relief surgery also poses certain surgical risks to patients. The complications may include:
- Pain in the area of the incision.
- Swelling and tenderness in the surgical area
- Headache or feeling heavy-headed as a result of the surgical incision and stitches
- Discomfort while sleeping, due to the stiffness of the muscles post-surgery
- Stiffness in the neck and inability to turn the head or neck for a few days
- Bleeding of the surgical wound very rarely occurs, given the minute incision that is done.
However, this is a potential risk of the surgery.
It must be remembered that all of these complications persist only for a temporary period of time. Once the surgical wound heals and the muscles & nerves recover, you will be able to get back to your daily routine. We recommend that you take plenty of rest and avoid exerting yourself for the first 3-4 days. Allowing the incision to heal will help you avoid post-surgical complications and infections. Pain medication is usually necessary for the first 3-5 days, due to incisional pain. Be sure to follow Dr. Rosenberg’s advice and speak to him if any of these symptoms persist for more than a week after the surgery.
Apart from the lessening or stopping of headaches, there is another positive result of the migraine relief surgery, in the form of the removal of frown lines. The corrugator supercilli muscles are known as the ‘frowning muscles’ and they create the frown lines on your face.
